    This year's event includes an optional field tour component on Tues (Oct 27). Please note participation in the field tour is reserved for those also attending Wednesday’s meeting. There isn't a field tour only option.

  • Participant Waiver and Release of Liability

    Event: Forest Health Review Field Tour 
    Dates: October 27, 2026
    Location: Simcoe County Museum (1151 ON-26, Minesing, ON L9X 0Z7)

    1. Assumption of Risk

    I understand that this course involves field-based activities, which may include tree planting, hiking and working on uneven or natural terrain. I acknowledge that these activities involve inherent risks, such as:
    • Trips, slips, and falls
    • Insect bites or allergic reactions
    • Exposure to sun, weather, or wildlife
    I freely and voluntarily assume all risks associated with my participation.

    2. Release of Liability

    I hereby release, indemnify, and hold harmless the Forest Health Review Partners, its employees, instructors, partners, and affiliates from any and all claims, demands, or causes of action related to any injury, illness, property damage, or other loss I may sustain while participating in this tour, including transportation to and from the site.

    3. Safety and Conduct

    I agree to:
    • Follow all instructions provided by leaders and staff.
    • Act in a manner that protects my own safety and the safety of others.

    4. Medical Consent

    In the event of illness or injury, I authorize the Forest Health Review partners and their representatives to arrange emergency medical treatment if deemed necessary. I understand that I am responsible for all associated costs. I agree to disclose any allergies, medical conditions, or limitations that may affect my participation.

    5. Photographic Consent

    I grant Forest Health Review partners permission to photograph or record me during the course and to use these materials for reporting, educational, and promotional purposes without compensation.


    I have read this Waiver and Release of Liability carefully, and I acknowledge and agree to be bound by its terms.
